i had knee issues for a while when i was new to tandeming -

usually is one or a combination of the following -

as you already mentioned, it could be that the seat is too high or too low 
or that more or less float is necessary in the pedals.  a third alternative 
that i have personal experience with is the cadence - we try to ride at 
about 90 and when climbing hills we shift way down so we can pedal easily. 
this makes a huge difference...no longer doing the monster mash on the 
pedals like i did as a kid seemed to do the trick....
