David and Nancy,

Many of us have had your frustrations, and Ralph and I would agree with all
the previous comments. By getting a good professional bike fit first, you
will be investing in your future riding enjoyment. Things that would not be
noticeable on shorter rides, become debilitating (and downright frustrating)
as your rides become longer and more frequent. As a good reference, here are
two links to an excellent detailed explanation, and description of bike fit
issues and options. These are (step by step) with breakdowns of lots of
different knee problems and solutions. If you spend some time reading
through each of them, you will have a much better understanding of how each
adjustment (or out of adjustment) can affect your knees. Reading through
this will also give you an appreciation of the value of someone who takes
the time to fit and refit each adjustment, because each time you move one
piece, it affects how your body fits with all the others.

Besides the:
 1.bike fit,  2.pedals with float,  3.higher cadence/lower gears on hills,
or,  4.just too much too soon 

Some cyclists turn their foot out far enough that even pedals with a lot of
float doesn't resolve it. It isn't the pedal so much, as it is that the hips
are wider and a shim on the crank arm that moves the pedal out farther away
from the frame just a little makes all the difference in the world. Women's
saddles are wider for a reason, but many people wouldn't know that the
distance between the pedals can be a factor too.

My wife and I are new to tandemming and ETC, and started training to do the
STP.   We've had to give that goal up this year because she has started
getting knee pain.   We would like to find a physical therapist or
orthopedist with enough experience dealing with cycling aches and pains to
help figure out which of the various kinds of knee problems this is and
advise on whether it would help to raise/lower the saddle or change to
pedals with more/less float.   Can any of you recommend someone in the
Seattle area?   Thanks,
